Please submit this form for all requests to change design configurations and product selections that have been approved and signed off on in a design meeting. We will note these changes on the meeting notes and ensure all revisions are made to capture the change. The design schedule may be modified if changes are made to the project scope, budget, or approved design work. Additional time required for changes will be billed hourly as an additional service.
