Please submit this form for all requests to change design configurations and product selections that have been approved and signed off on in a design meeting. We will note these changes on the meeting notes. Any major changes will affect the project cost and schedule, so we need this submitted formally to amend the agreement. Changes will be billed hourly per our rate schedule.
